The Tasmanian child sexual abuse commission of inquiry has referred more than 100 people to Tasmania Police and child protection since it began.
The commission's president revealed the numbers at the final hearing in Hobart of the Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings.
The commission focused on the launceston general hospital, Ashley Youth Detention Centre, out of home care and public schools.
"Although we believe that children are now safer in Tasmanian institutions than was the case in the past, more work needs to be done," commission president Marcia Neave said.
"We have continued to hear examples of poor systems, practices and cultures within government institutions even as our inquiry has drawn to a close."
Ms Neave said the commission had referred more than 100 people to police and child protection during the 29 months of the inquiry.
She said it was also the commission's view that the Ashley Youth Detention Centre should be closed as a "matter of urgency", and improving the safety of children in youth detention and out of home care should be a priority.
Ms Neave said the Tasmanian government's response to allegations and incidents of child sexual abuse since 2000 had too often been inadequate.
"While we saw pockets of good practice, this was often a result of the initiative and good judgement of individuals rather than something encouraged or enforced by a broader system.
"More commonly, institutions did not recognise sexual abuse for what it was and failed to act decisively to manage risks and investigate complaints."
The report, which contains 75 findings and 191 recommendations, is expected to be tabled in parliament on September 26, which will mean it will be publicly available.
"Preventing and effectively responding to child sexual abuse will require changes to laws and policies and also to social and institutional cultures and individual beliefs, attitudes and practices," Ms Neave said.
"If the necessary changes are not made children will continue to be subjected to child sexual abuse in these institutions," Ms Neave said.

Health department boss Kathrine Morgan-Wicks's tearful apology to abuse victims at commission of inquiry. The head of Tasmania's health department has made an emotional apology to the victim-survivors of child abuse within the health system, saying she was "personally horrified by the lack of empathy and humanity" shown by the department.
Department of Health Secretary Kathrine Morgan-Wicks also committed to making large-scale changes to ensure all complaints of child sexual abuse were acted upon in a trauma-informed way.
Tasmania's Commission of Inquiry into Responses to Child Sexual Abuse in Institutional Settings is holding hearings in launceston for two weeks and Ms Wicks - who became Secretary in September 2019 - appeared before Tasmania's on the final day of hearings on the health system.
Over the course of nine days, the commissioners have heard evidence from a range of victim-survivors who were abused by James Geoffrey Griffin, a paedophile who worked as a nurse on the children's ward at launceston general hospital (LGH) as well as accounts of other alleged abusers at the hospital.
It also heard about a litany of complaints, boundary breaches and red flags relating to Griffin that went nowhere.

The announcement comes after the Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings turned its focus to the launceston general hospital last week.
The inquiry has heard of catastrophic failings at the hospital around its handling of notorious paedophile James Geoffrey Griffin, who worked as a nurse on the children's ward.
During the hearings, it was revealed a senior executive at the hospital got most of her information about an investigation into Griffin from two podcasts.
The inquiry also heard Griffin's nursing supervisor said she was aware of him hugging young patients but his behaviour was "accepted by all staff" and in her opinion at the time something which could be addressed by "education".

Amanda Duncan thinks of a before and after when it comes to her big sister Zoe.

Her father rushed home to pick up her things, but on his return knew something was wrong
Zoe, aged 11, later alleged a doctor had raped her
Before 2001, Zoe was a passionate hockey player, wickedly funny, stoic in the face of a long list of chronic illnesses, and empathetic towards even dead flowers, which she would pick and arrange so they didn't feel left out.
After a mid-year admission to the launceston general hospital (LGH) - one of many in the then-11-year-old's life - everything changed.
"Zoe was a fun, charismatic, bubbly, happy-go-lucky girl, and she came out terrified, she would have night terrors, there was a lot of crying," Amanda said.
Zoe, suffering an asthma flare-up, had been left in the LGH emergency department while her parents dropped their younger daughter home.
Mr Duncan returned to find his daughter highly distressed, and said he knew something was wrong.
"It wasn't until several weeks later that she gradually revealed to a psychologist and to my wife the extent of what had actually occurred," Mr Duncan said.
Zoe alleged a doctor had raped her - a horrific claim her parents have long struggled to have taken seriously by those supposed to protect their child.
'Mum, will I be pregnant?'
Mr Duncan and his wife Anne remember the doctor's interest in their daughter when they arrived at the LGH emergency department.
They remember he told them she was beautiful, and how he looked forward to taking care of the young girl.

They remember he told Ms Duncan to take Amanda, who was six, home.
They remember he later told Mr Duncan to go home and gather Zoe's belongings for an overnight stay.
At first, Zoe told her father the doctor had squeezed her breast, told her he loved her, and stuck his fingers in her mouth, later telling the child to keep "their secret".
A hospital manager told Mr Duncan his daughter wasn't upset enough to have been sexually assaulted.
"He explained medically why those things might have happened," Mr Duncan said.
"Less than 12 hours after Zoe had reported to me what had occurred, I happened to bump into [another doctor] and she indicated to me she'd been to this doctor's house the previous night and she said 'the doctor and his wife are very upset, and the doctor's wife said the little girl must have misunderstood what the doctor was saying'.
"Then she went on to say that this doctor is a very nice man, and you need to hurry up and make up your mind about what you're going to do … I don't think he's going to take legal action [against you]."

Tasmania Police took a decade to investigate alleged paedophile nurse who worked with children, review finds
An internal review into how Tasmania Police handled its investigation into an alleged paedophile nurse has revealed the police were told of allegations he was abusing children as early as 2009.
It took another 10 years for police to formally investigate James Geoffrey Griffin, and only after a complaint was received by an alleged victim.
Even after police received the formal complaint, it took from early May until the end of July for his workplace, the launceston general hospital, to be informed of the allegations.
The review revealed the agency received information about potential child abuse in relation to Mr Griffin in 2009, 2011, 2013 and 2015.
The report highlighted issues with information-sharing between agencies, such as child protection and police.
For example, the 2013 report related to allegations of "inappropriate touching and grooming behaviour" by Mr Griffin.
Police referred the matter to child protection, who spoke to Mr Griffin and the alleged victim.
Both denied the allegations, so the file was closed and police took no further action. Police did not speak to Mr Griffin nor the potential victim.
As a result of the internal review, Tasmania Police said it had now implemented a specialist investigative and policy team to improve processes and procedures related to investigations into child sex abuse.
Mr Griffin took his own life in October 2019 after being charged with multiple child sex offences.
Tasmania Police Commissioner Darren Hine apologised to Mr Griffin's alleged victims for any harm caused by the deficiencies identified in the report.
"We will learn from this, we will make changes, we will do better for victims," he said.
Commissioner Hine said he wanted to reassure victims they could safely come forward and that their "matters will be pursued".
The report identified problems in information sharing across agencies, particularly with the Department of Communities, and called for a review of investigative guidelines of child sex offences.
Commissioner Hine said a specialist investigation and policy team has been established to address the issues raised in the report.
Premier Peter Gutwein said the government would provide an addition $1.5 million in funding for a historic complaints' review process lead by a specialist team within Tasmania Police, looking particularly at police and Department of Communities files.
"My expectation is that no stone be left unturned," Mr Gutwein said.
"I want to do this job once, I want to do it right, and I want to make sure I have the strongest protections in place."

Tasmania's child abuse inquiry has delivered scathing findings against the former head of medical services at the launceston general hospital over his handling of allegations against a notorious paedophile nurse and other medical personnel.
The commission of inquiry's final report into the Tasmanian government's responses to child sexual abuse was tabled in parliament on Tuesday.
The commission has made 75 findings and 191 recommendations designed to protect young people from abuse.
While most of the findings relate to institutions, it made five specific findings against Peter Renshaw, including misconduct.
"Dr Renshaw's omissions and fabrications amount to misleading our Commission of Inquiry," it stated.
"We do not make this finding lightly.
"Misleading a Commission of Inquiry undermines public trust and confidence in the process. Such an act by a senior state servant is unethical and unprofessional and brings the State Service into disrepute."
The commission described Dr Renshaw as an "unhelpful witness" who was "defensive and pedantic" during his appearance before the commission.
"Each of the concessions he made, once confronted by the evidence, had to be extracted from him during hearings," it stated.
"We consider that Dr Renshaw failed to accept responsibility for his failures.
"He did not demonstrate even a modicum of self-reflection during our hearings."
The commission said this "frustrated" victim-survivors and their families, who were seeking acknowledgement, reflection and apologies.
Failure to comply with mandatory reporting obligations, inquiry finds
One of the findings against Dr Renshaw related to a failure to comply with the hospital's protocol for the reporting and management of suspected child abuse.
This related to allegations made by 11-year-old Zoe Duncan in 2001 in relation to a doctor, who the commission has referred to by the pseudonym "Dr Tim".
The initial allegations were that Dr Tim had given Zoe a hug, kissed her hand, and said that she was a pretty girl and that if she was older he would marry her.
But despite being informed of the allegations, the commission said Dr Renshaw did not follow the protocols in place at the time.
"We consider that Dr Renshaw's failure to comply with the protocol - by failing to immediately alert the paediatric registrar of Zoe's allegations, his failure to discuss reporting to Child Safety Services with a senior colleague, and his subsequent interview of Zoe - may have contributed to delaying Zoe's disclosure of more serious allegations against Dr Tim, including that he had raped her."
The commission found Dr Renshaw's failure to comply with mandatory reporting obligations in a timely manner also "had an adverse impact on later investigations".

The commission's president revealed the numbers at the final hearing in Hobart of the Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings.
The commission focused on the launceston general hospital, Ashley Youth Detention Centre, out of home care and public schools.
"Although we believe that children are now safer in Tasmanian institutions than was the case in the past, more work needs to be done," commission president Marcia Neave said.
"We have continued to hear examples of poor systems, practices and cultures within government institutions even as our inquiry has drawn to a close."
Ms Neave said the commission had referred more than 100 people to police and child protection during the 29 months of the inquiry.
She said it was also the commission's view that the Ashley Youth Detention Centre should be closed as a "matter of urgency", and improving the safety of children in youth detention and out of home care should be a priority.
Ms Neave said the Tasmanian government's response to allegations and incidents of child sexual abuse since 2000 had too often been inadequate.
"While we saw pockets of good practice, this was often a result of the initiative and good judgement of individuals rather than something encouraged or enforced by a broader system.
"More commonly, institutions did not recognise sexual abuse for what it was and failed to act decisively to manage risks and investigate complaints."
The commissioners will hand their report to Governor Barbara Baker on Thursday. That will mark the end of the commission's work.
The report, which contains 75 findings and 191 recommendations, is expected to be tabled in parliament on September 26, which will mean it will be publicly available.
"Preventing and effectively responding to child sexual abuse will require changes to laws and policies and also to social and institutional cultures and individual beliefs, attitudes and practices," Ms Neave said.
"If the necessary changes are not made children will continue to be subjected to child sexual abuse in these institutions," Ms Neave said.
During Wednesday's hearing, the commissioners gave an outline of what their recommendations will include:
- Ensuring institutions know how to recognise warning signs of child sexual abuse, and how to respond to it
- Embedding safe, accessible and transparent complaints processes
- Age-appropriate sexual education delivered consistently each year in public schools
- More therapeutic services to ensure victim-survivors have timely access to supports
- More culturally appropriate Aboriginal healing services, as well as support services that know diversity and disability
- Breaking down silos between different government agencies and departments to ensure investigations into allegations of child sexual abuse are properly coordinated
- Giving those working with children the knowledge to recognise harmful sexual behaviours in children, and clarity about how they should respond to them
- Urgent short-term reforms to youth justice to "address the serious deficits in the care of children detained at the Ashley Youth Detention Centre"
- The development of a world-class, trauma-informed, rehabilitative approach to youth detention
- Dedicated leadership for out-of-home care, youth justice and child safety services within the Education Department, which has responsibility for all three
- Investments in out-of-home care, and transitioning from government to non-government out-of-home care providers
- A new commission of children and young people that would act as a "strong and fearless advocate for children's rights" and would have more powers than the current Children's Commissioner, including additional advocacy for children in out-of-home care and youth detention, and advocate for the needs of Aboriginal children and young people.

Child Safety Services (CSS) executive director Claire Lovell also used her time in the witness box at the commission in launceston to apologise to Tiffany Skeggs, saying CSS should have protected her.
The Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings has heard of catastrophic failings at the launceston general hospital around its handling of notorious paedophile and nurse James Geoffrey Griffin and the aftermath of revelations about him.
On Sunday - despite the commission's hearing still being underway - the Tasmanian government announced a review into management at the hospital, flagging the likelihood of "significant changes to leadership roles".
The case of Zoe Duncan
In 2001, 11-year-old Zoe Duncan was admitted to the launceston general hospital (LGH) following an asthma flare-up.
Her parents, Craig and Anne, earlier told the commission Zoe had disclosed to them that she had been abused by a male doctor during her stay there.
The Duncans told the hospital immediately - but CSS was not notified for nine days.
Police were only notified five months after the incident. It was the Duncans who reported it, believing the hospital would not.
Both investigations found her claims to be unsubstantiated.
On Monday, Ms Lovell was questioned about the investigation, which was undertaken before she started working for CSS.
Ms Lovell told the commission she was concerned that the report accepted the doctor's account over Zoe's, when the doctor was denying the allegations, despite not knowing their full extent.
She said there was no reason not to believe Zoe.
"It seems that she was making a consistent and clear disclosure that she'd been sexually abused. There doesn't seem to be a reason to discredit that," Ms Lovell told the commission.
"Why would anyone preference the account of an adult who's allegedly responsible for abuse, who has every reason to not be honest about that abuse and, in fact, is unwilling to even hear the details of what's been alleged?
"You can't normally place a lot of weight on denial of allegations of child abuse ... asking someone whether they have, or haven't, is pretty unlikely to result in the truth."
Ms Lovell told the commission that, while it did not mean there was enough proof for charges or convictions, she believed the report should have been substantiated.
"What happened wasn't right. It appears to me that Zoe Duncan was abused and this investigation didn't substantiate it and I believe that it should have," she said.
"That's a very poor outcome ... for Zoe and her family, and for that I'm extremely sorry that that was their experience. I'm so sorry that that happened."
Zoe died from epilepsy in 2017, aged 28.
She had refused to return to the LGH for treatment.

A victim-survivor who was abused by notorious paedophile James Geoffrey Griffin has told Tasmania's commission of inquiry that when she finally built up the courage to go to police, they told her they "couldn't charge a dead person".
The Commission of Inquiry into Child Sexual Abuse in Government Institutions has spent the past week and a half examining the failings in the health system that allowed Griffin, a nurse in the children's ward at launceston general hospital, and other alleged abusers to go undetected for almost two decades.
Multiple women who were groomed and abused by Griffin have given evidence over the course of the hearings.
Angelique Knight is the latest.
Ms Knight was 14 when she met Griffin during one of her stays at the children's ward at the launceston general hospital (LGH).
She told the commission she initially "hated him".
"I remember him coming in and he was very touchy-feely the first time he had even come into my bedroom, and I screamed out and I screamed that I wanted that man away from me," she said.
Ms Knight told the commission her mother made a complaint to the nurse in charge and said Griffin was not to look after her again, but nothing really changed.
He soon became her nurse again.
"Eventually I started to like him. I guess he just had me fooled very easily. We became close," she said.
Ms Knight said the touching started immediately.
"He was always touching me, hugging me, touching my bum, always had his arms around me ... even in corridors with other nurses around," she told the commission.
The touching eventually turned intimate.
She said staff at the hospital noticed, but no-one suggested it was inappropriate.
"I just always got told, 'that's just Jim, he's a touchy-feely kind of guy'," she said.
Reporting the abuse left her feeling worse
When Ms Knight got married in 2019, Griffin was supposed to give her away.
"It was planned for a long time. He was excited, he was over the moon," she told the commission.
"A week before something happened and he was told it was inappropriate and he couldn't give me away at my wedding ... he became my emcee."
By this stage, Ms Knight was in her late 20s.
"I was a bit more vulnerable when I was in LGH, then I became an adult, then it was my choice to see him. I hate myself that I kept going," she told the commission.
"I always just thought it was just me. If I had of said something when it started it could've stopped."
Ms Knight told the commission she found out about Griffin's death and his offending through the media.
She then went to police to report her abuse, but the reception she got left her feeling worse.
"It took a lot for me to go in there. When I went in there I spoke to a lady ... [she] spoke to a police officer and came back to me pretty quickly and said, 'sorry there's nothing we can do. We can't charge a dead person but sorry this happened,'" she told the commission.
"It's pretty serious for victim-survivors to get to that point to want to talk about it which is hard enough anyway ... then to be treated like 'why even bother?'
"I didn't feel like I was heard or anyone even acknowledged what happened."

When Kirsty Neilley first heard about the many crimes of notorious paedophile James Geoffrey Griffin, she struggled to reconcile that with the person she knew.

"I didn't believe that it could be real," she told the Commission of Inquiry into Child Sexual Abuse, which is conducting hearings in launceston about what happened at the hospital.
"That wasn't the person I knew."
Ms Neilley remembered Griffin as a "second father", a "family figure", a man who had attended her wedding.
But Griffin was not a family friend. He was a paedophile who worked as a nurse.
He met her when she was admitted to the children's ward at the launceston general hospital (LGH) in 2015.
She was a 16-year-old patient, he was her caregiver.
From early on he went out of his way to create a special bond with Ms Neilley, the commission heard.
"When I was first admitted, he was just like any of the other nurses," she told the commission.
"He'd look after me once or twice a week. Once we got to know each other better he was looking after me a lot more."
Ms Neilley was in there for mental health reasons and so always had someone watching her.
More often that not that person was Griffin.
She told the commission she was not sure if it was because they got on better or because he requested her.
Ms Neilley told the commission she began to open up to him and they started messaging on Facebook.
"He told me not to tell anyone," she told the commission.
Then late one night she got a phone call from him.
"He said somebody had reported him for getting to close to me," she said.
"I think the night nurse who was in charge had told Jim that he was to stop contacting me. So he gave me his phone number instead, so that it was just a random number coming up on screen instead of his Facebook name."
Unwarranted intimacy
She told the commission no one at the hospital spoke to her about their contact, and Griffin made it clear it was to remain a secret.
"He told me not to tell anyone because he could lose his job over it, but he didn't want me to be by myself," she said.
She also told the commission Griffin would give her long hugs and kiss her when she said goodbye.
"At the time it was totally like, I had no problem," she said.
"I felt like I had someone there for me. I don't know why he was doing it but I had no complaints, because he was there."
She said that, reflecting back, it wasn't the way she'd treat a family friend.
"Now I'm looking, I wouldn't treat any of my family and friends like that, I wouldn't give long hugs and kisses," she said.

The Tasmanian government is establishing its own review of a launceston hospital where a pedophile nurse was able to keep working despite multiple complaints.

Tasmania has launched a review into how a pedophile nurse had worked in a children's ward of launceston general hospital amid harrowing evidence from victims at an inquiry.
Premier Jeremy Rockliff on Sunday said the state government will establish a Child Safe Governance Review of the hospital and its human resources, to focus on the handling of serious misconduct such as institutional child sexual abuse.
"We will not be waiting to take action where it is clear it is needed. It is critical that there is a culture of accountable leadership in our hospitals," he said.
A commission of inquiry into child sexual abuse in the state's public service last week examined the LGH and the case of James Geoffrey Griffin.
Griffin worked at the hospital for almost two decades and took his own life in October 2019 after being charged with child sexual abuse offences.
Several survivors have told the inquiry about abuse by Griffin, who was the subject of multiple reports of breaching professional boundaries at the hospital.
The review will be led by the Department of Health's secretary and will be informed by a governance advisory panel of independent experts in child trauma, governance and hospital administration.
hospital staff and unions will be invited to participate as members to make recommendations, through a child safeguarding lens, on the hospital's management and leadership structures.
The move was welcomed by the state's medical professionals, with the Australian Medical Association Tasmanian president John Saul saying it was important to act sooner rather than later.
"We need to understand the ability of HR to act when a serious complaint is raised against an employee, while also balancing the need for natural justice principles to be upheld," he said.
The Australian Nursing and Midwifery Federation's Tasmanian branch secretary Emily Shepherd said while significant work will be required to improve systems and restore confidence, the review was a step in the right direction.
The review's recommendations will be handed to the department's secretary by November this year.

A "MEDICAL emergency" has interrupted a public hearing of Tasmania's child sexual abuse commission of inquiry, which unfolded during a tense examination of launceston general hospital's former chief executive Stephen Ayre.
It is understood Mr Ayre collapsed while giving evidence, and required hospitalisation.
Prior to the medical emergency, hospital nursing and midwifery director Sue McBeath told the commission she had a conversation in February 2021 with Mr Ayre - who was CEO between 2004 and 2008 - about his prior knowledge of paedophile nurse James Geoffrey Griffin.
She said that conversation came in the wake of Camille Bianchi's podcast The Nurse, which revealed Griffin's identity in the media for the first time and the extent of his crimes.
"During that period, Stephen had been the CEO and so I expressed my shock and absolute horror at what was in the podcast," Ms McBeath said.
"I said 'I knew nothing of this'. And he said that he had been contacted by the police during the time that he was the CEO and that on the advice of HR he was advised to let the police investigation take its course."
Ms McBeath - who previously held the role of nursing director overseeing the paediatric ward - said if she'd known Griffin was being investigated by police while she was in that role, she would have seen his "confused professional boundaries" as a newly qualified registered nurse as something "much more sinister".
"I suppose I was shocked there had been evidence at that time," she said.
"I would have seen (Griffin's) behaviours differently had I known there was a police investigation ... that's really regrettable that I didn't have that information. As a professional, duty of care is something we hold very strongly."
Mr Ayre is currently medical services executive director of the statewide Tasmania Health Service.
He gave evidence after Ms McBeath, strongly denying he had previous knowledge of Griffin's conduct from the time he was CEO of launceston general hospital.
"Did you disclose to her that you in 2021 had previous knowledge of Griffin's conduct?" counsel assisting the commission, Elizabeth Bennett SC, asked.
"Absolutely not," Dr Ayre responded.
"I told her that I hadn't listened to the podcast and that I was surprised by the allegations that had come forward as other people were."
He said he hadn't spoken with Ms McBeath "at all" about Griffin's conduct, not even what he'd subsequently learned about the paedophile in the wake of his arrest and death.
"I haven't followed any of the information that was forthcoming from his death in 2019," he said.
Ms Bennett suggested it was "an astonishing lack of curiosity" that Dr Ayre hadn't made enquiries about a paedophile nurse that had worked on the children's ward while he was CEO.
"I guess there was some interest in that but as I didn't have any knowledge of that prior to 2019, I did not take particular interest," Dr Ayre said.
"I had confidence that the system had actually addressed that issue."
He admitted he realised in 2019 there was "something to learn from the example of Griffin" - although he took no steps to learn about what had unfolded and made no enquiries.
Due to give evidence on Friday were Peter Renshaw, the hospital's executive director of medical services, who has received a number of child sexual abuse notifications during his decades-long tenure at the institution.
Also due to give evidence was Department of Health secretary Kathryn Morgan-Wicks.
The pair will now give evidence in August.
Ms Bennett told the commission the decision to adjourn was made given the effect of the medical emergency on those present in launceston.
She noted "many nurses and police officers" responded to the incident and did so "with enormous professionalism and care and concern".
The launceston hearings will continue until July 8.

The former chief executive of a Tasmanian hospital didn't make any inquiries about a male pedophile nurse who had worked at the facility's children's ward under his watch.
A commission of inquiry into child sexual abuse in the state's public service has this week examined the launceston general hospital and the case of James Geoffrey Griffin.
Griffin worked at the hospital for almost two decades and took his own life in October 2019 after being charged with child sexual abuse offences.
Stephen Ayre, who was the chief executive of the hospital from 2004-08, said he learned about "the issue" of Griffin "around the time of 2019".
"I'd have to say that I took a very peripheral interest because I didn't have any knowledge of it," Mr Ayre, the current director of medical services at the Royal Hobart hospital, told the inquiry on Friday.
Counsel assisting the inquiry, Elizabeth Bennett QC, asked Mr Ayre whether he made any inquiries between 2019 and 2021 about Griffin's conduct.
"I did not make any inquiries. I had confidence the issues were being dealt with and would be uncovered and we would have knowledge of the issues as they unfolded and information became available," he said.
hospital chief executive Eric Daniels on Thursday acknowledged "catastrophic" system failures in relation to how Griffin was handled.
Several survivors have told the inquiry about abuse by Griffin, who was the subject of multiple professional boundary breach reports at the hospital.
hospital social worker Kylee Pearn said she informed human resources at a meeting in 2011 that Griffin had abused her as a child but was told nothing could be done without a conviction.
No physical record of the meeting exists.
Friday's public hearing ended during Mr Ayre's evidence following a medical episode in the hearing room.
The evidence of executive director of medical services Dr Peter Renshaw and department of health secretary Kathrine Morgan-Wicks has been pushed back to August.
The inquiry was called in late 2020, largely in response to allegations against Griffin receiving media coverage.

Nurse reveals how paedophile always wanted to 'care' for teens
JIM Griffin was always eager to put up his hand to "care" for teenage girls at launceston general hospital's paediatric ward.
He was especially keen whenever the girls had mental health issues, eating disorders, or long-term chronic illness.
"The response was always 'that's just Jim'," nurse Maria Unwin said.
Ms Unwin worked with the serial paedophile nurse from when he started on the children's 4K ward during 2021.
While in charge on shifts, Ms Unwin started to feel "uncomfortable" with Griffin, saying she had "an uneasy gut feeling".
"He had a strong preference to care for teenage girls when it came to patient allocation," she told the child sexual abuse commission on Tuesday.
"He'd be very quick to put his hand up and say 'I'll take them', especially young girls with mental health issues or eating disorders or other long-term chronic illness, and sometimes younger children as well with complex backgrounds or illnesses."
Ms Unwin said Griffin liked to develop "a new best friend kind of relationship" with his young patients.
She said even though she had concerns, she felt she didn't have a clear complaint to lodge with the Australian Health Practitioner Regulation Agency.
Instead, she tried to allocate the patients he'd be interested in to other staff members.
Griffin wasn't pleased.
Once he made eye contact with Ms Unwin, "a very intimidating glare, as if to say 'why did you do that?'"
"He confronted me in the small kitchen ... and said 'why won't you let me look after them? Have I done something wrong?'" Ms Unwin told the inquiry.
"I felt intimated ... I responded with something like, 'it's somebody else's turn today'."
She said concerns had been raised with her manager, but "dismissed".
"There was a strong group of staff who really admired Jim. He was extroverted and confident, he was very friendly," she said.
"(Whenever I raised) my concerns with Jim, that something doesn't quite seem right, the response was always 'that's just Jim'. There was that support and that acceptance that that's just Jim."
Ms Unwin said coming forward to speak out put her career in the Tasmania Health Service at "very real risk" in being considered for future positions.
"In the end, I decided that I needed to do it for the victims and the families."
'Wet kiss': Complaints about paedophile nurse went nowhere
A WET kiss. An inappropriate conversation with a group of girls about a nurse he called "titsy", who he wanted to "shag". Two or more "deficient" reviews about a string of complaints. A transfer to a children's detention centre in the midst of a scandal.
Before 2019, when serial child sexual abuser James Geoffrey Griffin was finally charged, not one staff member at the launceston general hospital was trained in how to identify child sexual abuse or grooming.
Not one single person had oversight of child safety at the institution.
And there was no central repository where complaints of child abuse were handled.
On Tuesday, those "significant failings" that allowed child abuse and grooming to go unnoticed were admitted by the Department of Health human resources manager, James Bellinger.
Mr Bellinger admitted to the child sexual abuse commission of inquiry that prior to 2019, there was no child sexual abuse and grooming training provided to staff.
He admitted the hospital's incident reporting system, SLRS, was not designed to capture child abuse and grooming behaviours.
"Is it fair to say it took Griffin's offending to put this on people's radar?" Elizabeth Bennett SC, counsel assisting the commission, asked.
"Yes," Mr Bellinger answered.

WHEN serial child sexual abuser Jim Griffin was released on bail in 2019, one of his victims knew he'd soon be dead.
"I think from the moment I found out he'd been let out on bail, I just had this inward feeling that he's going to die," Keelie McMahon told the commission of inquiry on Wednesday.
"I just internally knew there was no way he would end up in jail."
Ms McMahon had finally come forward to the police about the abuse she'd suffered at Griffin's hands as a teenage girl - and her initial experiences with the force were positive.
But once Griffin died by suicide later the same year in the wake of a string of child abuse charges, her entire police complaint was simply "shut down" with no further investigation.
"I asked 'what happens from here? What do we do now? And I specifically remember him (the police officer) saying 'that's it, there's nothing we can do'," she said.
"That was it. I never heard from the police again. It felt like I'd put my trauma on the table and they'd picked it up and thrown it in the cupboard.
"They signed off on the paperwork ... and didn't want to deal with it anymore.
"There was no contact from the police, there was no contact from the hospital ... It was just radio silence - there was nothing."
Ms McMahon's mother - who had worked on the children's ward at launceston general hospital with the paedophile nurse - said she and her colleagues felt they were silenced after his death - with the matter "swept under the carpet".
"(Before) Griffin died ... when he left the ward at the end of July, we were sent an email to say Jim had retired, he will let us know when he wants a celebration, can we please respect his privacy, and he'll contact us when he feels like he's able to," Annette Whitemore told the commission.
"When he died ... there was another email to say a colleague had died over the weekend.
"There was no mention of who it was, and then it went straight into confidentiality as nurses, 'please don't talk about it'."
Ms Whitemore said the fact she and her colleagues felt silenced made the situation more traumatic.
"We were told not to talk about it. That is not how you treat trauma," she said.
Feeling their concerns and complaints were shut down after Griffin's death, the mother and daughter spoke to interstate journalist Camille Bianchi in her podcast, The Nurse.
Ms Whitemore felt it was more important for the truth to come out than to protect her career at launceston general hospital.
"I was prepared to lose my job, I found another job to go to."

Helen Bryan has given evidence at Tasmania's Commission of Inquiry into Child Sexual Abuse, which is this week focusing on paedophile James Geoffrey Griffin, who was a nurse on the children's ward for almost two decades.
Ms Bryan is an executive director of nursing at the LGH.
She told the commission of inquiry she heard about Griffin in July 2019, when the executive director of medical services, Dr Peter Renshaw, told her that there was a police investigation into allegations that he was a paedophile.
Ms Bryan told the inquiry she agreed that it was a critical incident - the discovery a paedophile had been working on the ward for 18 years - but that it wasn't responded to, as such, and that she should have been actively involved, but wasn't.
She said she had been excluded from "multiple meetings".
"That doesn't dissolve my responsibility and accountability, and I accept that, and I would do things very differently if this happened tomorrow," she said.
"I do want to apologise that I didn't properly, or I committed to properly fulfil my full responsibilities, and I let others exclude me from the process."
Ms Bryan told the commission that Dr Renshaw took over the running of the hospital's response and review following the 2019 revelations about Griffin.
"I didn't see a report. I had no further input or feedback," she said.
"I got a lot of my information from the two podcasts that I listened to."
Journalist Camille Bianchi's podcast, The Nurse, was one of the catalysts for the commission of inquiry.
Ms Bryan conceded she should have sought out more information.
The chief executive of the hospital, Eric Daniels, told the inquiry that, when he found out about Griffin, he was out of his depth. He "unreservedly apologised" for the failings.
"I was not prepared for an event like this. I'd never encountered this problem previously and I don't think I was prepared to manage it effectively."
He told the commission that there had been catastrophic failures at the hospital over its handling of Griffin.
'Complaints system inadequate
Another senior manager conceded that none of the hospital's complaints systems was adequate to identify child sexual abuse, and no one had had the training needed to identify such abuse or grooming.
Janette Tonks is the hospital's nursing and midwifery director of women's and children's services, and Griffin's direct manager reported to her.
The commission of inquiry has this week heard that were multiple reports and warnings given to Griffin about professional boundary breaches while he worked at the hospital.
Ms Tonks told the inquiry that reports could be made through a patient liaison complaint form, by going straight to the nurse unit manager or through a system called SLRS, which was designed for clinical safety reports.
However, she said, none of those systems could satisfactorily identify child sexual abuse, and there was no central repository to identify instances or potential flags for child sexual abuse or grooming at the hospital.
"Subsequently, I'm aware that none of us had any education and training in child sex abuse or grooming behaviours," Ms Tonks said.
Ms Tonks told the inquiry that she had not heard anything until this week about a 2011 report made to the hospital by Kylee Pearn, who earlier told the commission that she had warned the hospital that Griffin was a paedophile and had abused her.
Ms Bryan told the inquiry she also had not heard about Ms Pearn's report until today.
"That's absolutely unacceptable and ... I should have known about it. I didn't know about it," Ms Bryan said.

Senior launceston general hospital (LGH) figures have told an inquiry that staff had no training about how to identify grooming, and that current nurses may not be fully aware of mandatory reporting requirements.
A commission of inquiry examining Tasmanian government responses to child sexual abuse allegations in the public service is this week focusing on James Geoffrey Griffin.
Griffin took his own life in October 2019, shortly after being charged with multiple child sexual abuse offences.
Several survivors have told the inquiry they were abused by Griffin, including Tiffany Skeggs, who met him at her netball club, where he was a volunteer.
Griffin sexually abused and emotionally controlled Ms Skeggs from the age of 12 and through her teenage years.
The inquiry has been told Griffin breached professional boundaries at the LGH multiple times and was warned, but the matters weren't escalated.
In 2011 a colleague of Griffin's disclosed to LGH human resources that she had been sexually abused by him as a child, but the "matter went nowhere".
Eric Daniels, chief executive of the hospital, agreed there had been a catastrophic failure of LGH structures and management.
"With the information I've received this week, it's not robust," Mr Daniels said on Thursday.
"It doesn't provide for an appropriate amount of accountability. It doesn't provide for … ensuring the safety of children in our care is appropriate."
He said he didn't order an investigation into Griffin after his death, and was told by subordinates that workplace conduct allegations made against Griffin had been investigated and not substantiated.
Mr Daniels agreed the only reason he knew more about Griffin's conduct was because the inquiry had investigated.
"Why did you not ask for there to be a robust investigation when you found out there had been a pedophile on a children's ward?," counsel assisting the inquiry, Elizabeth Bennett, SC, asked.
"I can't answer that I'm sorry," Mr Daniels replied.
Nursing and midwifery director Janette Tonks said that before 2019 no one at the hospital had been trained to identify grooming.
The hospital's executive director of nursing, Helen Bryan, said she could not guarantee every nurse currently employed was fully aware of a mandatory requirement under law to report child sexual abuse suspicions.
Ms Skeggs, who came forward to police in 2019, said she was later told by a detective Griffin had been on their radar for some time.
Griffin died by suicide while on bail.
"I cannot for the life of me fathom how anyone, whether it was the initial police bail or the subsequent court bail, could deem he was safe to return to the community," Ms Skeggs said.
"I had made police aware … of him stating 'I will f*cking kill myself before I go to prison', and they still released him."
The inquiry was called in November 2020, largely in response to allegations against Griffin receiving media coverage.

Sonja Leonard was Jim Griffin's boss for an entire decade.
The pair had a friendly working relationship.
For years, the launceston general hospital children's ward nurse unit manager saw how Griffin hugged his young patients and referred to them as "baby", "babe" or "princess".
Griffin also committed a number of "boundary violations" over the years - like cuddling a child on a recliner and giving his phone number to patients.
But the complaints system at the hospital was unclear - and often undocumented.
The first port of call on the paediatric ward 4K was to notify Ms Leonard.
But as she admitted before the child sexual abuse commission of inquiry on Wednesday, she had "a negative relationship with a lot of staff on the ward".
Ms Leonard also admitted the culture on the paediatric 4K ward was "complicated" and "dysfunctional", and that there was a practice of keeping complaints "in-house".
She said she "couldn't recall" whether it took until 2017 - just two years before Griffin was charged with numerous child sexual abuse offences, and then died by suicide - to tell him the behaviour was inappropriate.
At that point, Ms Leonard drafted a letter with the support of the hospital's human resources department to inform Griffin "this is not the first instance of a complaint of this nature" and "external agencies have been made aware".
Ms Leonard began crying as she claimed her team had been "manipulated" by a paedophile who took advantage of a ward in disarray.
"The culture and the conflict and the undermining that was going on in the ward at the time, unfortunately, were a perfect storm for Mr Griffin to take advantage of," she said.
"I have a lot to learn, as we all do.
"I feel deep, deep, deeply that we were deceived, we were manipulated, and we were sold a version of Mr Griffin that he wanted us to believe."
Also on Wednesday, former human resources consultant Luigino Fratangelo said he couldn't remember meeting with whistleblower and hospital social worker Kylee Pearn in 2011 when she raised the alarm that Griffin was a paedophile.
Mr Fratangelo said he couldn't remember if he advised Ms Pearn that Griffin couldn't be removed from 4K unless he had a conviction.
But he admitted if he had met Ms Pearn, he would have told her to go to police, and that he understood under the state service code of conduct, suspending a staff member required a conviction.
The inquiry also heard that Griffin was the Australian Nursing and Midwifery Federation delegate for the 4K ward for 16 years.
Tasmanian branch secretary Emily Shepherd told the commission the union had "no knowledge" of his offending until after his 2019 death.
"It was absolutely shocking to the ANMF that an ANMF delegate had these allegations against him," she said.
The commission of inquiry will continue its launceston hearings until July 8.

Nursing staff at the launceston general hospital were told their colleague had retired and to respect his privacy, when he had actually been suspended because of child sexual abuse allegations, Tasmania's commission of inquiry has heard.
Keelie McMahon and her mother Annette Whitemore have given evidence at the inquiry into child sexual abuse in government agencies, which is this week focusing on the launceston general hospital (LGH) and James Geoffrey Griffin, the now notorious paedophile who worked on the children's ward of the hospital for almost two decades.
Ms Whitemore was a registered nurse and worked with Griffin, and their families became friends and went on camping trips together.

Ms McMahon told the inquiry she was sexually abused by Griffin from when she was 14, around late 2011, but did not tell anybody.
At the end of July 2019, a police investigation into Griffin was underway because of other complaints, but Ms Whitemore said an email was sent to staff at the LGH saying "Jim had retired".
She said the email stated that "he will let us know when he wants a celebration, can we please respect his privacy and he'll contact us when he feels like he's able to."
"I thought the bit in the email 'respect his privacy' was a little bit odd and staff that had worked with him for 18 years sent him messages to see how he was.
"And then it all came out."
Ms McMahon said she didn't tell anybody about her abuse until her mother told her that someone else had made a complaint about Griffin.
"My first thought went to, people aren't going to believe her," Ms McMahon told the inquiry.
"To begin I didn't want to make a formal statement but then I thought about it and I thought 'no, I need to do this for the other people more so than myself'."
Ms McMahon made a police complaint in September 2019.
Ms Whitemore was still working at the hospital, and said she told her managers what had happened and took some time off.

In 2011, Kylee Pearn told the launceston general hospital that there was a child abuser working in the hospital's children's ward.
She knew the nurse was a paedophile, because years earlier, she had been abused by him.
"I just knew I had to do something… I felt he was a risk on that ward," she said.
But Tasmania's Commission of Inquiry into Government Responses to Child Sexual Abuse has heard that her complaint went nowhere.
James Geoffrey Griffin continued to work on Ward 4K for another eight years, racking up more and more complaints until he was finally charged by police.

Kylee Pearn had known James Griffin since she was four years old - he was a family friend.
She told the inquiry, which is holding hearings in launceston, that he started sexually abusing her when she was about seven years old, and that continued on a regular basis for a number of years.
Her coping mechanism had been to lock away the abuse; it was something that had happened, but it was not who she was, and she doesn't like to label herself a victim-survivor.
As an adult, Ms Pearn worked as a social worker in family violence support, and then in 2011 she began a secondment at the launceston general hospital (LGH), working on the same floor as Griffin.
"I would see him coming and going and ran into him a few times," Ms Pearn told the inquiry.
"It was incredibly confronting, and on a couple of occasions he approached me and kissed me on the cheek."
Despite her coping mechanism of locking away what had happened, Ms Pearn told the commission that two events that year made her feel the need to report Griffin.
Firstly, a chance conversation with a friend who revealed she too had been abused by Griffin.
"That was a very big realisation for me, that I wasn't the only one," Ms Pearn said. "Because that's something I'd told myself for a long time, it was probably just me."
Then, Ms Pearn's own child had to spend a night on Ward 4K.
James Griffin was working, and she was too petrified to leave her child alone.
"I just thought how incredibly unfair it was that I could protect my child, but no one else in this ward knew that information," Ms Pearn said.
"It wasn't just about me, it was about my profession, being a social worker, being a mum, wanting to protect other kids on that ward."
She told her manager, Stewart Millar, about her abuse and they arranged a meeting with the hospital's Human Resources department.
"I felt he was a risk of abusing other children."
Ms Pearn hoped he would be removed from the children's ward.

Tasmanian authorities allowed a pedophile nurse to work with children for 18 years after complaints were first raised about him, leading to the sexual abuse of multiple children.
This damning evidence was presented to Tasmania's royal commission-style inquiry into state agencies' response to child sexual abuse on Monday.
Counsel assisting the inquiry, Elizabeth Bennett SC, outlined a litany of failures by police and health authorities to take action against James (Jim) Geoffrey Griffin, who abused children while working at launceston general hospital from 2001 to 2019.
Ms Bennett told the hearings in launceston that the state's anti-corruption body, the Integrity Commission, referred a complaint about mishandling of the Griffin complaints to the same bureaucrats responsible for the failures.
The commission also heard of wider child abuse by staff at the LGH, including from abuse survivor Ben Fenton, who gave evidence he was assaulted by a man referred to in proceedings as "George".
There was also evidence 11-year-old girl Zoe Duncan, since deceased, was assaulted and raped in 2001 at the LGH by a doctor referred to in proceedings as "Dr Tim".
Ms Bennett said a man who purchased a computer from Jim Griffin found child exploitation material on the device and reported this to Tasmania Police in September 2000.
After not hearing back from police, the man emailed them in March 2001 to say: "I find it very distressing that I have heard nothing about any inquiries. I do not want to think that he (Griffin) is working in a kids' ward somewhere in Tasmania, given what I have found."
That is exactly what Griffin was doing. Ms Bennett said Griffin started work at the LGH in February 2001 and in September 2001 began work on pediatric ward 4K.
"He remained working on that ward, with some exceptions, for the next 18 years (until charged by Police with multiple child sex offences in 2019)," she told the commission.
Ms Bennett detailed a series of concerns and complaints about Griffin raised by staff and parents from 2004 to 2017. Only in 2019, following an abuse allegation to police, was a full police investigation launched. Griffin killed himself in October 2019 while awaiting trial.
Both Mr Fenton and Zoe Duncan's parents, Anne and Craig Duncan, were critical of the handling of their complaints by Peter Renshaw, LGH director of clinical services since 1987.
Mr Duncan told the commission Dr Renshaw "downplayed" an initial complaint, including that Dr Tim had touched his daughter's breasts, and had not taken the matter seriously.
Dr Renshaw provided a statement to the commission saying he had thought the complaint - which at that stage did not include rape - was a "professional boundaries issue".
However, he now accepted he should have "mandatorily reported" the complaint earlier and apologised for this failure.
Mr Duncan said the rape and failures to adequately respond to it had completely changed Zoe and contributed to illnesses that caused her death in 2017. "The stress and the hurt... literally killed her," he said.
The commission continues.

A "notorious" alleged pedophile nurse in Tasmania was reported to police and breached boundaries with patients across almost two decades before he was criminally charged.
Tasmania Police was provided with child exploitation-related evidence about James Geoffrey Griffin 19 years before he was charged with sexually abusing children in 2019, an inquiry heard on Monday.
There were multiple occasions where Griffin overstepped boundaries with young patients while working at the launceston general hospital's paediatric ward 4K.
A commission of inquiry into Tasmanian government responses to child sexual abuse allegations in the public service is for the next fortnight examining the health system.
Griffin took his own life in October 2019 not long after being charged with child sexual offences.
In September 2000, a person told Tasmania Police they found child exploitation material and links to child abuse websites on a computer they had purchased from Griffin, then a student nurse.
In March 2001, the person sent a follow-up email to police, counsel assisting the inquiry, Elizabeth Bennett SC, said during opening submissions.
"I find it very distressing that I have heard nothing about any inquiries," the email read.
"I do not want to think he is working in a kids ward somewhere in Tasmania unsupervised, given what I have found."
Ms Bennett said the inquiry had been unable to determine the outcome of the complaint.
Griffin started at ward 4K in September 2001.
He received a written warning about a boundary breach in 2004 after hugging a child, allegedly kissed a child on the head in 2005, and in 2009 offered to stay overnight with a young female patient
In March 2009, police received a report Griffin had "upskirted" young girls while working as a medic on the Spirit of Tasmania ferry service.
His house was searched, with police noting he cleared his computer browser history daily and had a "large number" of photos of young girls.
Ms Bennett said the matter was "filed for intelligence".
"There is no record of formal notification from Tasmania Police to the launceston general hospital at this time," she said.
In March 2015, Tasmania Police was provided with "credible information" that Griffin had discussed child abuse online.
"A source provided (further) information to police in April 2015 … it appears the information was not accessed and no further action was taken," Ms Bennett said.
Tasmania Police representatives are set to provide evidence to the inquiry in coming weeks.
In 2017, the hospital noted Griffin called a patient pet names, resulting in a third written warning that matters could be escalated if his behaviour continued.
Police allegedly found child abuse material at Griffith's house in mid-2019 after a survivor made accusations against him.
He was suspended from work and charged with several counts of indecent assault, possessing child exploitation material and sexual intercourse with a young person.
"The case of James Griffin is one of rumour, notoriety and fear," Ms Bennett said.
Tasmania Police in 2021 issued a formal apology for deficiencies in their investigation into Griffin.
Survivor Ben Felton said he was abused by a different male nurse at the hospital in 1989 as a 13-year-old and his complaints fell on deaf ears.
The inquiry was told 11-year-old Zoe Duncan, who suffered epilepsy and died in 2017, was raped by a doctor at the hospital in 2001.

Angela said Lillian had separation anxiety when she was younger but had been making progress before she went to the LGH in 2018.
Now, she said, Lillian was reluctant to go anywhere without her mother, and Angela no longer takes her to the LGH for medical treatment. Instead, she drives to Hobart.
Angela said she planned to move.
"I only want to stay here for the next couple of years and then I want to take [Lillian] somewhere else ... I believe [Lillian] will live a happier life somewhere else with her sisters."
For years, Griffin worked as a registered nurse at the Paediatric Centre at the LGH, the Spirit of Tasmania ferry and at the Ashley Youth Detention Centre.
He died by suicide in October 2019, a month after he was charged with a number of child sexual abuse offences.
The first alleged abuses dated back to the early 1980s, and, according to an internal Tasmania Police review, the first allegation against Griffin was made in 2009.
Angela said it was "a big reality check" when allegations of child sexual abuse against Griffin were reported in the media.
"Everything made sense from our last day [at the hospital]. It was a big shock," she said.
The commission of inquiry is holding six weeks of public hearings in Hobart and launceston over the coming months.
Its particular focus areas are the health and education departments, launceston general hospital, Ashley Youth Detention Centre and the state's out-of-home care system.
When asked what message Lillian would have for the commission, Angela said:
"That everyone's got a voice and you've just got to listen, people have to listen."

A mother phoned Tasmania's Child Safety Service because she said she did not know who else to call when she was increasingly worried about what was happening to her daughter at the launceston general hospital.
Angela* gave evidence to the Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings on Tuesday.

She told the commission her daughter, Lillian*, who has a disability and is non-verbal but able to communicate, was a vibrant "people person" whose life was changed forever when she spent time in the children's ward at the LGH in 2018 when she was about 11 years old.
It was the same ward where paedophile nurse James Geoffrey Griffin worked for years.
Angela said Lillian started "acting strangely" around Griffin.
"When I asked him about a rash that had started, he wanted to see it and then when I slightly pulled her nappy down ... he's come over and he's tapped her with his hand and said, 'She'll be right.'"
Angela said Griffin was touching Lillian on the vagina and did not make any attempt to communicate with her. She also said hospital staff generally did not attempt to communicate with Lillian.
She said when she told Griffin that Lillian was able to communicate, he was "very shocked".
"Over the next couple of days was when things got worse," Angela said. "I turned up the next day and she was screaming in her bed sweating.
"The blinds were all pulled down, and this was the room right outside the office of the ward.
"I've just barged in there and seen her, and I've got her up and gave her a cuddle and then I gave her a shower, and she had cream everywhere all over her vagina ... it was just plastered on there, it wasn't normal, it's not right."
Angela said she noticed Lillian had an injury to her vagina.
"I was running in and out of the hospital crying because I just could not understand what was going on with her and the doctors would not listen to me, no one was listening to me, no nurse was telling me what was going on, who was putting cream on her, nothing like that."
Angela said that was why she called Child Safety Services.
"I didn't know who else to call."
She said she asked Griffin about the cream.
"He just shrugged it off, they all shrugged it off," Angela said.
She said the head nurse told her she could make a complaint.
"She said she could give me a form to put in and that's what she did, and she told me she was going to take complaints but whether or not it happened, I have no idea."
Angela said no-one had ever followed up with her, and she felt that the hospital staff looked down on her because she was a single parent.
"They called me at one stage, 'Oh the girl with the Ugg boots', you know, that's how they treated me ... they've always been glad to see the back of us for some reason."

hospital and medical staff "shrugged off" concerns an 11-year-old quadriplegic girl was touched on the vagina by a male nurse and had developed a vaginal injury, an inquiry has heard.
Giving evidence on Tuesday to Tasmania's Commission of Inquiry into state government responses to child sexual abuse, the woman said she made a complaint about the 2018 incident but never received a response.
She said "everything made sense" when the nurse involved, James (Jim) Geoffrey Griffin, was later charged by police with multiple child sex offences and exposed in the media.
"Everything that we assumed had happened, happened - and if not worse," she said.
Despite this, when her daughter was admitted to the launceston general hospital (LGH) with a staph infection, staff made no attempt to set up a method of communication.
The woman told the commission that, reflecting on that failure now, "it makes me feel sick, and horrible and angry".
Praised by commissioners for her "courage" in giving evidence, the woman said she noticed her daughter was "very distressed" while on LGH ward K and started "acting strangely" around Griffin.
At one point, she slightly pulled down her daughter's nappy to show Griffin a rash that had developed. Griffin responded by "tapping" her daughter's vagina with his hand and saying "she'll be right".
"When I told him that she could use an eye gaze (to communicate) he was very shocked," the mother told the commission. " ... He's gone 'oh, so she can communicate can she?' "I've said 'yes'.
"I turned up the next day and she was screaming in her bed, sweating and the blinds were all pulled down.
"She had this cream everywhere, all over her vagina ... She had an injury down there. She had skin off down the bottom of her vagina and she was really distressed ... It wasn't normal. It wasn't right."
The hospital staff looked down upon her as a single parent, calling her "the girl with Ugg boots".
Griffin and other staff "all just shrugged it off" when she raised concerns. She complained to the head nurse but "she didn't seem that fussed". A complaint form was submitted but no one had ever responded.
On her return home, her daughter had been "very distressed for a long time". "I just asked her if the man was bad, and she said 'yes'," she told the commission.
Her daughter had been set back by the experience, with a return of separation anxiety. "She's always going to be like that now in a hospital setting," she said.
She could not return to the LGH. "She'll start yelling as soon as we go through those (hospital) doors," she said. She now had had to drive her daughter to Hobart for treatment.
She hoped the commission would result in more focus on the suitability of staff dealing with children and greater respect for children with disabilities. "Everyone's got a voice; you've just got to listen," she said.
Tasmania Police has apologised for failing to lay charges against Griffin until 2019, despite receiving information about his alleged crimes in 2009, 2011, 2013 and 2015. Griffin killed himself in October 2019 while facing multiple charges.

Alleged child abusers kept in teaching, youth detention roles 'for months'

Teachers subject to child abuse ?allegations have been allowed to continue working for up to nine months, while a "concerning number" of youth detention staff have been stood down over claims.
These were among the revelations on Monday from the first day of full hearings before Tasmania's commission of inquiry into state government responses to child sexual abuse.
The inquiry, equivalent to a royal commission, heard that one schoolgirl was groomed and ?assaulted by a teacher, and later also groomed and possibly abused by notorious launceston general hospital pedophile nurse Jim Griffin.
Counsel assisting the commission Elizabeth Bennett SC foreshadowed potential adverse findings against several people, and recommendations for systemic reform.
"In a number of case studies teachers continued to teach ?despite being the subject of allegations of child sexual abuse," Ms Bennett told the commission in Hobart.
"(This raises) questions not only in relation to the department's processes, but those of the Teachers Registration Board and the registrar for working with vulnerable people.
"In one case, a relief teacher continued to receive postings for nine months after concerns were first raised about his conduct ?towards children, which were echoed in subsequent postings."
Ms Bennett said the commission would hear first-hand from victim-survivors sexually abused by teachers in public schools.
"One hypothesis to be explored is whether, in responding to allegations of child sexual abuse through a disciplinary lens, the ?department has overlooked its quite separate duty of care to its students," she said.
Evidence before the commission raised concern that "even today" children at the ?Ashley Youth Detention Centre west of launceston were "at heightened risk of sexual and physical abuse".
"The systems in place to prevent and respond to that abuse may not be fit for purpose," Ms Bennett said. "A concerning number of staff at Ashley have been stood down as a result of allegations made against them by former detainees.
"Some have been stood down since this commission commenced its work. Others have moved on to other positions, ?others have left the state service."
Ms Bennett said some youth detention staff had "apparently been permitted to remain at Ashley in the face of repeated allegations about their conduct".
"Why was the state apparently so slow to stand down some staff members?" she said. "Why have some children been exposed to harm through being inappropriately placed in units with other ?detainees who pose a risk to them?"
A witness known as Kim gave evidence that her Grade 9 daughter was groomed for months by a teacher, who then took her driving and kissed her on the lips. Her daughter, known to the commission as Paula, received a letter from the teacher suggesting the incident was "her fault" for not running away or screaming at his advances.
The incident contributed to her daughter's anxiety and eating disorders and led her to self-harm. She believed Paula, who had since died, was later groomed by Griffin, whose crimes were not revealed until after Paula's death.

About the Commission
The Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings was established on 15 March 2021 by Order of the Governor of Tasmania.
The Commission is equivalent to a royal commission and is independent of the Tasmanian Government.
Under its terms of reference, the Commission's inquiry will focus on the adequacy and appropriateness of the Tasmanian Government's current responses to allegations and incidents of child sexual abuse in institutional contexts.
The Commission will submit its report and recommendations to the Governor of Tasmania by 1 May 2023.

Tasmania's Premier Peter Gutwein has spoken for the first time about wrestling free from a teacher who sexually assaulted him when he was 16 years old.
Mr Gutwein was speaking at the end of a COVID briefing on Friday outlining changes to mask rules.
He was asked by a reporter about whether he would discipline members of his own party who were heard groaning during Question Time in response to a question about the state's upcoming Commission of Inquiry into child sexual abuse.
The Labor Opposition Leader quoted a question from a Tasmanian victim-survivor of child sexual abuse who said the Premier had shown "no empathy" on the issue.
At the press conference this afternoon, Mr Gutwein said he had trouble sleeping last night.
"I must say last night this really troubled me, to have a victim-survivor that felt this government was not being supportive," he said.
He then said he was going to reveal something only once: "I am not going to speak about it again."
"I have great empathy with people who are victim-survivors. I crossed the floor in 2002 to bring about a Commission of Inquiry that ultimately could have completely derailed my own career," he said.
"I'm the first premier of this state that has taken the steps to have a Commission of Inquiry.
"The reason that I've done that is that I have great empathy, because I have walked in their shoes.
"When I was a young man, when I was 16, I was the victim of a sexual assault. It was by a former teacher.
"I was asked to go to their home for a beer, which I thought was fantastic.
"I'd known this person for a long period of time, they'd spent a lot of time at the football club I was involved with.
"While I was there, they put their arm around me and they grabbed for my groin, tried to get their hand down my pants.
"I was big enough and strong enough to get them off me and get away."
Mr Gutwein said he had not "spoken to anyone about the motivations about why I crossed the floor in 2002".
He said he had reached out to the victim-survivor who took offence at the groaning of government MPs.
"I hope they get in touch," he said.
"I know what the loss of trust feels like, I know what the shame feels like."
What happened in parliament to spark this?
Mr Gutwein's revelation comes after Labor leader Rebecca White yesterday read out a question in parliament on behalf of a victim-survivor of notorious paedophile nurse James Geoffrey Griffin.
Government frontbenchers were heard groaning and saying "come on" as Ms White spoke: "The victim survivor who watched you yesterday said she saw you show no warmth, no empathy or even mention the victims."
"I beg your pardon," Ms White said after the interjections.
"I am directly quoting a victim-survivor. How insulting."
Mr Gutwein later stood up in parliament to acknowledge his MPs had caused offence.
"On behalf of the government, and without explanation or excuse, I wish to unreservedly apologise," he said.
The Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings is examining many allegations of abuse in state settings, including by Griffin.
Griffin was a paediatric nurse at launceston general hospital for 19 years.
He was charged in 2019 with child sexual abuse offences and took his own life.
The commission held its opening hearing on October 26 last year.
It will recommence its hearings in May, with hearings to be held in Hobart and launceston.

Tasmanian health department put interests of alleged pedophile James Griffin ahead of victims, Ombudsman says

Tasmania's Ombudsman has accused the state's health department of putting the interests of an alleged paedophile nurse ahead of his victims in a scathing review into its response to requests for information.
The Tasmanian Health Service's (THS) handling of allegations against James Geoffrey Griffin - who took his life in 2019 after being charged with child sex offences - has been singled out for special attention in the Commission of Inquiry into child sex abuse.
But behind the scenes, the department has argued it would be against the public interest to release details of complaints against Griffin while he worked on the launceston general hospital (LGH) children's ward - despite Ombudsman Richard Connock finding many documents requested by freelance journalist Camille Bianchi early last year could be published in some form.
In April 2020, Bianchi applied under Right to Information laws, for records of staff complaints, reports and email correspondence relating to Griffin's behaviour while working on Ward 4K.
More than 100 pages of relevant information were unearthed - but the department claimed none of the documents could be released, citing privacy issues, concern for the mental health of Griffin's friends, family and former colleagues, and fears publicising such information could deter people from making similar reports in the future.
A decision published by Mr Connock about a month ago disagreed.
"Allegations against Mr Griffin were never formally proven and were denied by him, and there are valid procedural fairness concerns in releasing documents regarding disputed allegations and unfinalised disciplinary and criminal proceedings," Mr Connock wrote.
"The weight of these factors is reduced, however, due to the extensive publicity already given to the allegations against Mr Griffin and his death … the damage has already been done."
The THS argued LGH staff were suffering stress and anxiety over the "matters associated with Mr Griffin", feelings compounded by guilt and grief.
"While the department's consideration of the interests of its staff and Mr Griffin's associates is understandable, I am concerned that it does not appear to have considered the interests of the victims of Mr Griffin's alleged offending while he was in its employ and the concerns of LGH patients and the general public about the adequacy of management of concerns by the department as highly," Mr Connock said.
"I consider that the public interest in protecting the interests of alleged sexual abusers of children is lower than that of the victims of such abuse.
"In contrast, the department does not once mention or appear to consider the victims of Mr Griffin's alleged offending or the valid community concern and desire for accountability from the department, given that abuse is alleged to have occurred against vulnerable child patients receiving care in a public hospital over an extended period."
The documents have still not been released, but the Ombudsman's account of the records shows relevant documents included:
- Handwritten notes used to record incidents of concern
- A formally reported "incident" in August 2017
- A notification to the Australian Health Practitioner Regulation Agency
- An email chain between a Tasmania Police officer and a THS doctor "regarding whether further action should be taken"
- Mr Griffin's resignation letter
Bianchi, who pushed ahead with podcast The Nurse despite the THS's refusal to cooperate, said the Ombudsman's review was enlightening.
"As distressing as this process has been, it's offered an invaluable insight into the contempt with which the system treats the public," she said.
"This information belongs to the patients and staff at LGH, and many others whose lives have been impacted by the matters raised in these documents."
Five women contacted Tasmania Police with complaints of sexual abuse in the months leading to his death.
The first woman to contact police alleged Griffin had sexually abused her from the age of 11.
According to the coroner's report into his death, Griffin made admissions of sexual misconduct in relation to her.
Police also found "a significant amount of child exploitation material", including "self-generated images" after searching his home.

The problem of child sexual abuse in Tasmanian public institutions is much greater than isolated incidents of predators gaining access to children, those gathered at the first day of a new inquiry have heard.
Speaking at the opening of the Commission of Inquiry into the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings in Hobart on Tuesday, counsel assisting the inquiry, Maree Norton, said the work of the commission would have four main focus areas; public schools, hospitals, youth detention, and out-of-home care.
"While each case study will have its own issues, we anticipate that certain themes and lines of inquiry will transcend individual institutions," Ms Norton said.
She continued that the "information available to the commission invites the conclusion that child sexual abuse within Tasmanian government institutions has consisted of more than just isolated incidents of predators gaining access to children within an otherwise safe system".
"Rather, we are concerned that such abuse may have been ... made possible by structural weaknesses by how these institutions understand and respond to child sexual abuse."
Ms Norton said that, "at worst there may have been an active culture of cover up or minimisation to protect reputations and institutional interests".
Commission president Marcia Neave said the commission would largely focus on current responses to child sexual abuse, and provide an "honest assessment" of the Tasmanian government's responses.
Ms Neave said the commission would aim to ensure perpetrators were held accountable wherever possible.
She said the commission had so far obtained 21,000 documents.
Government schools
Education Department documents associated with a civil court case show two teachers who were the subject of numerous complaints, and who were later convicted of child sexual abuse, were moved from school to school.
Ms Norton said the commission would look at whether complaints were referred to police and whether children and their advocates were listened to.
Public hospitals
The focus will be on the launceston general hospital where paediatric nurse James Geoffrey Griffin worked for 19 years.
He was charged in 2019 with child offences.
Ms Norton said complaints were made against him as far back as the early 2000s "and periodically thereafter".
She also said the commission had received allegations of "similar patterns of abuse" at the Royal Hobart hospital.
How the complaints were handled, and whether there was a deliberate cover up of abuse will be examined.

The number of alleged victims of an accused launceston paedophile male nurse has grown to four, with two new complaints, including an allegation of rape.
Two further writs and statements of claim were lodged in Tasmania's Supreme Court on Friday, by two young women who allege they were abused by paediatric nurse James Geoffrey Griffin when they were teenagers.
One alleges Griffin - who killed himself in 2019 after being charged with multiple child sex offences - sedated and then raped her at the launceston general hospital.
Both women - aged 17 and 14 when the alleged abuse occurred - accuse the Tasmanian Health Service of failing to protect them and of failure to act on prior allegations against Griffin.
"This is another shocking example of the alleged abuse by Griffin, of young women who are vulnerable and defenceless," said the women's lawyer, Kim Price.
"Our clients, and many other young people, have suffered because the hospital has not done its job properly."
Mr Price said more writs were expected, with Griffin accused of abusing multiple girls over a number of years.
One of the new complainants alleges Griffin abused and raped her at the LGH in 2011, when she was receiving treatment following an anaphylactic reaction.
"Griffin gave the plaintiff pain medication in the nature of sedatives until she was barely able to stay awake, then raped her while incapacitated in a private room at the hospital," documents lodged with the court allege.
"Griffin helped the plaintiff into the bath, washed her back, head, hair, legs and then her chest area including her nipples, groping her breasts.
"He washed between her legs and stroked her genitals, separated her legs and digitally penetrated her, which caused extreme pain."
Griffin continued "a consistent and predatory course of conduct" during the remainder of the then 17-year-old's treatment, including cuddling her, touching her thigh and rubbing her arms and legs, and calling her pet names.
The Tasmanian Health Service is yet to lodge a response to the claims. Premier Peter Gutwein and Tasmania Police have both apologised for "shocking" failures to adequately investigate and deal with complaints against Griffin.
These are likely to be further highlighted by a state commission of inquiry into government agency failures in response to child sex abuse allegations.
"We continue to hear that numerous complaints were made to launceston general hospital about Griffin - patients, staff and parents were speaking out for years," said Mr Price, of law firm Arnold, Thomas and Becker.
"Despite these complaints, we allege that Tasmanian Health Service failed to intervene and enabled Griffin to continue to work at the hospital and to use it as his hunting ground."
The women are seeking damages and medical bills, in separate legal claims.

Two women who claim they were sexually abused as teenagers by an alleged pedophile nurse are suing the Tasmanian government for failing its duty of care.
The pair were patients in launceston general hospital's paediatric centre in varying stints between 2015 and 2018.
They allege James Geoffrey Griffin, who killed himself two years ago after being charged with child sex offences, groomed and abused them at the hospital and on sleepovers and camping trips
In a statement of claim lodged with the Supreme Court of Tasmania, one woman, who was in hospital with an eating disorder, alleges Griffin called her "baby girl", "darling" and his "special girl".
She claims Griffin sexually abused her while taking observations, including measuring blood pressure, and changing her into hospital gowns.
She was also allegedly abused by Griffin at a motel.
He alleged told her "don't tell anyone", "that's what friends do" and "this is our little secret".
According to court papers, she complained numerous times to the nursing manager about Griffin but the hospital "did not accept the complaints or act on the request to remove Griffin's access to (her)".
Griffin is accused of deliberately befriending the second girl, who was also in hospital for an eating disorder, to lower her inhibitions, and spending time alone with her in a private room.
"Griffin was forceful and intimidating, and told (her) not to tell anyone," her claim reads.
The hospital did not act on complaints made by the girl's mother, it is alleged.
Both women are suing for damages and medical bills in separate legal actions, arguing the Tasmanian Health Service owed them a duty to exercise reasonable care.
They say they suffered serious harm, including post traumatic stress disorder and major depression.
Law firm Arnold Thomas and Becker say they plan to lodge "many further claims" and two more are likely to be made this week.
Allegations against Griffin will be examined by the Tasmanian Government's Responses to Child Sexual Abuse in Institutional Settings inquiry, which is expected to hold hearings later this year.
The royal commission-style inquiry was set up by the state government in late 2020 after allegations against Griffin and workers at the Ashley Youth Detention Centre came to light.

An internal review into how Tasmania Police handled its investigation into an alleged paedophile nurse has revealed the police were told of allegations he was abusing children as early as 2009.
It took another 10 years for police to charge James Geoffrey Griffin, and only after a complaint was received by an alleged victim.
Even after police received the formal complaint, it took from early May until the end of July for his workplace, the launceston general hospital, to be informed of the allegations.
The review revealed the agency received information about potential child abuse in relation to Mr Griffin in 2009, 2011, 2013 and 2015.
The first report to police came from an interstate police agency alleging Griffin had taken photos of children in a public place.
Police investigated that report by executing a search warrant and speaking to Griffin, but found no evidence of an offence.
Tasmania Police's review found it handled those allegations appropriately.
The 2013 report related to allegations of "inappropriate touching and grooming behaviour" by Mr Griffin.
Police referred the matter to child protection, who spoke to Mr Griffin and the alleged victim.
Both denied the allegations, so the file was closed and police took no further action. Police did not speak to Mr Griffin nor the potential victim.
The 2015 report related to a referral from the Australian Federal Police related to Griffin and sexual offending and child exploitation material.
"Deficiencies in the management of this information by Tasmania Police have been identified and are the subject of a current Professional Standards investigation that relates to the Police Service Code of Conduct," the review said.
As a result of the internal review, Tasmania Police said it had now implemented a specialist investigative and policy team to improve processes and procedures related to investigations into child sex abuse.
Mr Griffin took his own life in October 2019 after being charged with multiple child sex offences.
Tasmania Police Commissioner Darren Hine apologised to Mr Griffin's alleged victims for any harm caused by the deficiencies identified in the report.
"We will learn from this, we will make changes, we will do better for victims," he said.
"I think this has fallen short of everyone's standards, we need to make sure we continue to learn and evolve in relation to these matters."
Commissioner Hine said he wanted to reassure victims they could safely come forward and that their "matters will be pursued".
The report identified problems in information sharing across agencies, particularly with the Department of Communities, and called for a review of investigative guidelines of child sex offences.
Premier Peter Gutwein said the government would provide an addition $1.5 million in funding for a historic complaints' review process lead by a specialist team within Tasmania Police, looking particularly at police and Department of Communities files.
"My expectation is that no stone be left unturned," Mr Gutwein said.

Last month Marcia Neave was announced as the president for the Commission of Inquiry into allegations of sexual abuse in government institutions.
Tasmania's equivalent of a Royal Commission, the broad-ranging inquiry will have the power to hold hearings in public, to call witnesses and to demand the production of documents.
It replaced three inquiries that had already announced: into allegations of abuse in the Health Department, the education department and in the Ashley Youth Detention Centre.
Premier Peter Gutwein acknowledged the podcast, however refused to make comment on whether the nurse was still working in Tasmania's health system.
"I read and heard of Mr Felton's circumstances over the last 24 hours. Firstly can I just say it's brave he has brought these matters forward," he said.
"In terms of these matters, the one thing I'm certain that all of the brave victims that have raised these matters want to be assured of is that the government does not engage in discussion about matters that might ultimately prejudice justice being served."
Labor's Shadow Minister for Child Safety, Sarah Lovell, said the latest allegations further demonstrate the State Government's secrecy.
"There is already an unacceptable level of secrecy surrounding the 13 people who to date have been stood down in the public sector, pending further investigation into abuse claims," she said.
"The Premier, Peter Gutwein, and his Ministers have not adequately explained why they are refusing to release details of the agencies accused individuals worked in."
The Australian Nursing and Midwifery Federation Tasmanian Branch said they were offering full support to the matter being thoroughly investigated and addressed.
"The ANMF were not aware of these allegations when they were made 31 years ago, however, we support Ben and any individual in raising any instances of abuse or inappropriate conduct with Tasmania Police," said ANMF Branch Secretary, Emily Shepherd.
"ANMF members on Ward 4K have been instrumental in having the Commission of Inquiry established to examine child sexual abuse at the launceston general hospital and other Government institutions."

SENSATIONAL claims of cover ups and corruption in one of Tasmania's major hospitals have come to light, as a man breaks his silence over another alleged paedophile nurse.
Ben Felton, 44, was abused by a nurse in launceston general hospital as a teenager, according to independent journalist Camille Bianchi, in her podcast The Nurse, where last year she revealed the horrific crimes of James Geoffrey Griffin.
Griffin was charged with a dozen child sex abuse offences after 30 years working at the hospital. He committed suicide shortly after he was charged.
Mr Felton was 13 in 1989 when he was admitted to the hospital's paediatric ward with a case of pneumonia.
"This was back when they didn't have rooms, it was just one big room ... about 11.30 at night I asked the nurse who was on shift for a drink of water," he told the podcast.
"(The nurse) took me into the hospital pantry or kitchen ... and gave me a glass of water. He then started feeling my abdomen and asking me a lot of questions.
"He then pulled my pants down and he touched me while he was smiling."
Mr Felton alleges he slapped the man's hand away and "bolted".
Sources in the podcast allege a deal was made with Ben's parents after the young boy told his father what happened.
A senior hospital employee 'Steve' allegedly promised Mr Felton's family the nurse would be reprimanded and his permanent record would be marked.
'Steve' allegedly quashed any claims of child sex abuse in the ward, according to the nurses.
The alleged paedophile nurse was "shuffled" away, according to Ms Bianchi - but another ex-employee said the man tried to get a job in the ward again 12-13 years later and remains employed in Tasmania's health system.
There are also claims the man worked in other institutions with children including the Scouts.
After years of red tape and frustration attempting to follow up what happened to him as a child, Mr Felton finally acquired hard copies of the nurses reports from 1989.
"So now I've got the records and it wasn't a figment of my imagination and it did get brushed under the carpet ... (Steve's) probably forgotten all about this, but I certainly haven't," Mr Felton said.
Since the claims last year the State Government has stood down 13 public service employees in response to sexual abuse or misconduct reports.
However it refuses to release further details about any allegations until claims are fully investigated.

Tasmania youth detention staff stood down over abuse allegations
Three staff at Tasmania's youth detention centre have been stood down following allegations of abuse that state parliament has heard include a claim of rape made against a staff member.
Greens leader Cassy O'Connor told parliament on Thursday a staff member at the Ashley Youth Detention Centre had been stood down and their Working with Vulnerable People registration suspended.
"There are hundreds of allegations made against this person, including one of rape," Ms O'Connor told the House of Assembly. "We also have information that other staff at Ashley have been stood down."
She called on the state government to detail how many allegations were pending and how many staff at the centre, in northern Tasmania, were "implicated".
"Our information is that the senior manager who has been stood down is not the only staff member at Ashley who has been stood down ... but importantly, there may still be staff working at Ashley today who have allegations of a serious nature made against them," she said.
"We are talking about Tasmania's most at-risk young people. Voiceless young people who've been detained at AYDC, a manifestly failed model that is failing young people ... and that is failing to keep our community safe."
Human Services Minister Roger Jaensch later confirmed three people at the troubled centre had been stood down after "historical allegations were referred to police for investigation".
"The employees were stood down and an independent investigation is now underway, in addition to the police referral," he said. "The government needs to allow this process to take its course before commenting further."
Tasmania Police said it was "assisting Communities Tasmania with an internal inquiry". "At this stage no formal complaint has been received," TasPol said in a statement.
"Tasmania Police encourages anyone who is the victim of criminal activity to make a formal report to police who will assess the information and investigate, where appropriate."
Ashley has long been the centre of controversy, with claims of abuse, poor conditions, excessive workers' compensation claims and debate over the cost and appropriateness of incarcerating children.
A legal class action alleging physical and sexual abuse at Ashley, dating back to the 1970s, has reportedly attracted more than 120 claimants.
An alleged pedophile nurse, James (Jim) Geoffrey Griffin, who committed suicide last year after being charged with multiple child sex offences, worked at Ashley for five months in 2017.
The government has insisted no allegations were made about Griffin at Ashley. It has announced an inquiry into Griffin's decades-long tenure as a pediatric nurse at the launceston general hospital, but is resisting widespread calls for a commission of inquiry.

Tasmanian government investigating how alleged pedophile James Griffin went undetected
Alleged pedophile paediatric nurse James Griffin has been condemned by the Premier who supported the Health Minister's calls for another investigation into how the abuse went undetected for 28 years.
When asked whether a formal inquiry would be forthcoming Premier Peter Gutwein said, "further steps will be considered in due course, following the completion of the review".
Health Minister Sarah Courtney labelled the allegations "absolutely appalling" and said she was disturbed by them.
When Griffin was charged in October 2019 the Tasmanian Health Service stood him down and Ms Courtney said "an investigation was then undertaken".
"We are making sure that this matter is examined fully. I have asked the secretary to make sure that the matters around this both currently and historically are reexamined to ensure that Tasmanians can have confidence," Ms Courtney said.
"There is nothing more important than the safety of our children."
Ms Courtney said she had asked the health secretary to ensure the support offered to staff at the launceston general hospital was maximised.
"The secretary and I are committed to ensuring that these matters are taken extremely seriously," Ms Courtney said.
"Any patient or member of staff who has any concerns regarding a staff member's behaviour are encouraged to come forward, and I have been assured all matters will be properly examined and all appropriate support provided."
Australian Nursing and Midwifery Federation Tasmania branch secretary Emily Shepherd said the ANMF was unaware of the allegations until after Griffin was deceased.
Ms Shepherd said the investigation was a positive step and was confident the investigation would restore the confidence of ANMF members and the public.
"The information I have from the secretary of health is there will be robust measures taken," Ms Shepherd said.
Labor spokeswoman Rebecca White said, "it is incredibly concerning that this behaviour continued for so long without it being properly investigated and this person being removed from the workplace".

Ordinarily, one might expect that the story of a paediatric nurse killing himself directly after being charged with molesting multiple children would make huge headlines around the country.
But even in a town ripe with gossip, the case has been all but hushed up.
"This is a story you don't know, unless you live in launceston" says Bianchi, "and even then, it's been whispered to you in the pub, or passed only by word-of-mouth.
"People are scared. Scared to lose jobs. Scared to upset those who are covering up what's happened."
Griffin was connected in various circles of power and the hospital has so far not co-operated with the investigation.
"Nothing about this has been straight forward. The story that was first brought to me was that these women wanted to speak and no media was interested or willing to touch it. People had tried, but it always fell over," says Bianchi.
"I'm not the first journalist to know about it. Multiple journalists do. But local media is often under-resourced and the ramifications of publishing something like this are very different in a small community where everyone knows each other, compared to a capital city.
"In regional media you can't burn your sources and when you start to call some of these big institutions to account and lay hefty allegations, or even just ask too many questions, you can put a target on your back.
"You can come under pressure from editors not to burn bridges because you'll be cutting off a future pipeline of information and stories, not just for yourself, but you may in fact be jeopardising your entire team's access to politicians and information."
Protecting an investigation against leaks is also extremely difficult in a small community, as probing questions have a way of tipping off subjects, and can even trigger defamation proceedings if the questions themselves are deemed libellous.
"If you don't know something and you're trying to firm it up in good faith you have to be very, very careful in how you do it," says Bianchi. "Many journalists tried to do this story, but it always ended up in the 'too hard' basket, because local newsrooms just don't have the bandwidth."
News.com.au and Bianchi have now both obtained the coroner's report and the original police charge sheet, but even that took months to obtain.
A further Freedom of Information request (known as Right to Information in Tasmania) has so far been completely blocked by Tasmania's Health Department.
The request was first submitted in April 2020. It asks for any information relating to complaints about James Geoffrey Griffin, along with various other information.
The request yielded a schedule of 101 documents, but all 101 have been redacted with the department citing 'privacy concerns'.
However what that schedule of redacted documents does confirm, is that launceston general hospital was in receipt of complaints against Griffin dating back to at least 2009. When Bianchi and news.com.au contacted the federal health watchdog, AHPRA, to determine whether the complaints were ever passed along to them, as required, that line of questioning revealed they had no record of the hospital ever having forwarded the 2009 complaint, or another complaint which had been lodged in 2017.
"This was a serial premeditated predator," says Bianchi. "James Griffin planned how and when he could abuse young children - and put himself in the best possible situations to do it: he preyed on sick kids. I don't understand how or why complaints weren't forwarded, or why we are being stonewalled now.
"The victims want this information released. The public will want to know. It is most certainly in the public interest to release those 100 plus secret documents."

'IT'S LIKE VOLDEMORT, YOU AREN'T ALLOWED TO SAY HIS NAME'
On October 3 charges were laid. But then, 10 days later, Griffin attempted to take his own life. He was found on October 14 in his home, sitting in a chair. There was paperwork and bills organised on his dining room table - as if he was putting his affairs in order - along with family photo albums and sealed letters addressed to his family containing cash.
But James Griffin was not dead.
He'd threatened to kill himself earlier, even telling one victim he would "die before going to prison."
Griffin was rushed to the same hospital where his own colleagues were required to try to revive him.
"Some had heard rumours by then," says podcast host Camille Bianchi "but most had no idea.
"A lot of the staff had been 'groomed' by him too. They'd seen him befriend patients on social media, or give them backrubs, or even carry naked little girls back from the shower. They might have raised an eyebrow, but he was so convincing: he was part of the furniture, a trusted friend, someone who made everyone believe he was just a 'father figure' willing to extend an extra arm of support to vulnerable families like single mums, or patients with mental health issues."
But they were also beginning to learn of the charges and the events which triggered his suicide attempt.
"How can you be that person and be so evil at the same time? I thought he was genuine," says one nurse on the condition of anonymity.
"He groomed a lot of us," says another. "I think he showed a different side of himself to different people."
And then, suddenly, he died.
On October 18 in launceston general hospital, Griffin was pronounced dead. A coroner's report would later find that "when he took the drugs which caused his death he did so with the express intention of ending his own life voluntarily and alone. No doubt the charges he was facing at the time of his death motivated his action."
On Ward 4K though, nurses were in shock as gradually news of both the death and the charges began to leak out.
The whole ward was "genuinely traumatised", says Bianchi.
"They felt like they lost him twice. The day he died, and then the days later when they found out who he really was.
"They were grieving both Jims: the loss of the Jim they knew and the horror of the one they were just beginning to meet."
Speaking to Bianchi, one nurse commented: "It's like Voldemort, you aren't allowed to say his name".
HE ROBBED US OF OUR DAY IN COURT
Of course, the nurses weren't the only ones impacted.
"I didn't know how I was meant to feel or how I was allowed to feel," says Keelie.
"Jim was charged with indecent assault against me and then he died. That was it. It all just happened so quickly: it would have been within a month [of reporting and] it was over."
Keelie and the other survivors are now frustrated that the court system failed to hold him in jail or prevent him from taking the coward's way out.
"His suicide was a 'f*** you' to the victims and to the whole process," says Bianchi.
"He took the easy way out. He deprived his victims of their day in court and the chance to look their predator in the face and say 'this is how you hurt me'.
"These people - the ones he hurt as children, the ones who came forward and reported him to police - they were just beginning to find their voice and agency again. They had waited years. They wanted answers. But he was released by the courts without adequate supervision and now they will never get justice.
"There will be no answers, no resolution, no validation of their suffering. When he died the cogs of justice just creaked to a halt. It was 'case closed'."

A serial paedophile who spent almost three decades grooming, drugging and molesting vulnerable children - also filming and bragging about his crimes online - has been unmasked as a paediatric nurse who worked for 18 years on the children's ward at launceston general hospital in Tasmania until 2019.
Last October, police in launceston laid more than a dozen charges against James 'Jim' Geoffrey Griffin, a 69-year-old man from Legana, who since 2001 had worked on Ward 4K, the paediatric unit attached to launceston general hospital.
The charges relate to sexual offences against children as young as 11 - abuse which Griffin admitted to during police questioning last year. The details of some crimes are so disturbing that news.com.au cannot publish them all.
The seven-page charge sheet, obtained exclusively by news.com.au, spans 28 years, with sex crimes dating back to at least 1987, when Griffin worked as a volunteer ambulance officer for the Tasmanian Ambulance Service. The sexual crimes continue through to 2019, at which time Griffin was working at the hospital while also volunteering as "medic" and "masseuse" for a local children's sporting team.
On his arrest in October last year, police also discovered a "significant amount" of child pornography downloaded from the internet, as well as child pornography he had self-created. Some included children from the hospital ward.
On internet chat sites, detectives then uncovered examples of Mr Griffin "bragging" to others, outlining how he would use various drugs to sedate little girls "in order to sexually abuse them."
Now, nurses from the hospital are breaking their silence, revealing how he would use the ward as his own personal hunting ground to "groom" and "target" sick and vulnerable kids. In some cases, he even preyed upon the children of his own colleagues.
The explosive revelations have been unearthed by investigative journalist, Camille Bianchi, who is set to expose all in a compelling true crime podcast called The Nurse.
'BRAGGING HOW HE WOULD DRUG YOUNG GIRLS TO SEDATE THEM'
Keelie McMahon was about seven years old the first time she met Griffin at a hospital Christmas party which she was attending with her mother, Annette, a fellow nurse on the children's ward.
"They used to do Christmas parties for the 4K Ward and obviously quite often he'd be there," says Keelie, now aged 23.
"My little sister was about two, I think, so I would have been about seven and that's the first Christmas party I remember meeting Jim at."
For years, Annette and her family trusted Griffin, spending weekends and sharing vacations with him and his family.
"I was about 14 [when he] started to groom me," says Keelie.
"It was just showing me a lot more attention than he would most people. He used to hug everyone so that was nothing unusual but he started getting a bit more hands on, longer hugs. I remember him introducing me as his 'special girl'. If we were staying at his house he'd offer to give us back rubs to help us sleep. But I still tried to convince myself he was just being Jim, he was just being friendly."
That grooming escalated and one night in 2011, while 14-year-old Keelie was attending a sleepover at his place, Griffin came up to where she was laying.
"There were a fair few of us there watching movies and then once we decided to go to sleep, that's when he decided to take it a bit further with me."
With other children sleeping close by, Griffin brazenly sexually assaulted Keelie.
